Insurance and Financial Policies
- When your examination is completed, we will ask for payment of your office visit, deductible or co-payment of your insurance benefits. Payment is expected at the time of service unless previous arrangements have been made.
- We accept most insurance carriers when a medical condition exists. Please tell us about any secondary or supplemental coverage.
- Some insurance plans have a separate Vision benefit for routine eye examinations. Since we do not participate with all Vision plans, please call to inquire about your specific coverage.
- Some insurance plans require prior authorization from your primary care physician. Please obtain this referral prior to your visit so that your examination is not delayed.
- We will file your insurance claims as a courtesy, if we participate with your insurance.
- Forms of payment include check, cash, MasterCard, Visa, American Express and Discover.
